
If you’ve been using Claude Chat or Claude Cowork for more than a few days, you’ve probably hit a wall that made you want to throw your laptop out a window. The conversation suddenly gets confused. It starts repeating itself. It forgets what you told it five minutes ago. It gives you a cheerful response that completely ignores what you actually asked.
None of this means AI is broken. It means you’ve hit the edges of how it works, and once you understand those edges, you can work around them instead of fighting them. This guide covers the most common issues I’ve run into, what’s actually happening behind the scenes, and how to fix it (or prevent it in the first place).
“It forgot what I told it earlier in the conversation”
What’s happening: Every AI conversation has a context window, which is essentially how much text it can “see” at once. Think of it like a desk. The more papers you pile on, the more likely something slides off the edge. In long conversations, early messages literally drop out of what the AI can reference.
What to do: If a conversation is getting long and Claude starts losing track of earlier context, it’s time to start a new conversation with a handoff doc (more on that below). You can also try restating the important context in your current message: “Just to recap, we decided to do X and Y. Now I need help with Z.”
Prevention: For complex projects, keep a running document (a .md file in your Cowork folder works great) that summarizes key decisions and context. Reference it explicitly: “Read the project-notes.md file before responding.”
“It keeps apologizing and repeating itself”
What’s happening: AI models are trained to be agreeable, which sometimes means they get stuck in a loop of acknowledging your feedback without actually changing behavior. They’ll say “You’re absolutely right, I apologize for that” and then do the exact same thing again.
What to do: Be very specific about what’s wrong and what you want instead. Don’t say “That’s not right, try again.” Say “The tone is too formal. Rewrite this paragraph in shorter sentences, using ‘you’ and ‘I’ instead of ‘one should.’ Keep it under 50 words.” Direct, concrete instructions break the apology loop.
Prevention: Add something like this to your global instructions: “If I say something’s off, just fix it. No need to apologize or explain what went wrong. Just give me the corrected version.”
“It gave me a confident answer that was completely wrong”
What’s happening: This is called a hallucination, and it’s the most important limitation to understand. AI doesn’t “know” things the way you do. It generates text that sounds plausible based on patterns. Sometimes those patterns produce something that sounds right but isn’t. It can cite studies that don’t exist, quote statistics that are fabricated, and present speculation as fact, all while sounding completely sure of itself.
What to do: Always verify claims that matter. If Claude says “a study from Harvard found that…” go find that study. Ask Claude to provide links or specific citations you can check. If it can’t, that’s a sign the claim might be generated rather than sourced.
Prevention: Add this to your instructions: “When you cite a statistic or study, include the source. If you’re not confident a fact is accurate, say so rather than presenting it as certain.”
“The conversation went off the rails”
What’s happening: Sometimes a conversation accumulates enough confusing context that Claude can’t recover. Maybe you changed direction several times, or gave contradictory instructions, or the conversation just got too tangled. At a certain point, it’s fighting against its own earlier responses.
What to do: Start fresh. Seriously. Open a new conversation, write a clear prompt that establishes what you’re working on, and begin again. It’s almost always faster than trying to rescue a confused conversation.
Prevention: When you feel a conversation starting to drift, pause and restate the goal. “Let’s reset. Here’s what I’m trying to accomplish…” A mid-conversation reset is much easier than a full restart.
“Cowork created a file but it’s empty or broken”
What’s happening: Cowork sometimes runs into issues with file creation, especially with complex formats like .docx or .xlsx. The AI might think it successfully created a file when the actual output is corrupted, empty, or incomplete.
What to do: Always open and check the file immediately after Cowork creates it. If it’s broken, tell Claude specifically what’s wrong: “The file is empty,” “The formatting is gone,” “Only the first page has content.” Usually a second attempt works.
Prevention: For important documents, ask Claude to describe what it created before you open it: “Describe the contents and structure of the file you just made.” If the description doesn’t match what you expected, catch it before you waste time with a broken file.
“It ran out of context mid-task”
What’s happening: Context windows are large but not infinite. If you’re working on a big project (like editing a long document or analyzing a large dataset), Claude may run out of room to hold all the relevant information plus the conversation history.
What to do: This is exactly when you need a handoff doc. Summarize where you are, what’s been decided, what’s left to do, and start a new conversation with that summary as the opening context.
Prevention: Break big projects into phases. Instead of “Edit my entire 50-page report,” do “Edit chapters 1-3, then we’ll do the rest in a new conversation.” Plan the handoff points before you start.
“It keeps using a tone or style I don’t want”
What’s happening: Without explicit style guidance, Claude defaults to a generic helpful-assistant voice. If you want your voice, you have to teach it.
What to do: Provide examples. “Here’s a paragraph I wrote that sounds like me. Match this tone.” Or create a brand voice document and reference it in every conversation.
Prevention: Put your voice preferences in your global instructions. Something like: “Write in warm, conversational prose. No bullet points in emails. No corporate jargon. Use parenthetical asides.”
